The South Jersey Health Collaborative, consisting of the Burlington, Camden, and Gloucester county health departments, along with Cooper University Health Care, Jefferson Health, and Virtua Health, invites all community partners to join a discussion about findings from the 2025 Community Health Assessment and to help shape the Community Health Improvement Plan (CHIP) for South Jersey.
We value your participation, ideas, and feedback as we work together to enhance the quality of life for everyone in South Jersey. Thank you to those who have already contributed significantly to this process. This forum will foster meaningful discussion to create a collaborative plan for collective action in South Jersey. Your voice is essential, and we encourage you to attend and share your perspectives.
Attendees will include a diverse group representing health and human service providers, social, civic, and faith-based organizations, neighborhood leaders, advocates, planners, policymakers, community champions, and residents throughout South Jersey.
Cooper will host the forum to share findings on June 18, from 8:30 a.m. to noon, at the Cooper University Hospital Conference Center. Space is limited, and registration is free!
