Population Health enhances patient outcomes and quality metrics, lowers cost of care, and improves provider satisfaction throughout the care continuum. Each quarter, Population Health identifies an employee who exemplifies its values – Innovation, Patient Empowerment, Commitment to Excellence, Compassionate Care.
Last quarter, Katrina Donato, Care Coordinator, was recognized for her innovation when working with a patient after a hospital admission. All employees are encouraged to nominate someone who drives Population Health outcomes.
