AHRQ Medical Office Patient Safety Culture Survey

Starting December 8, 2014, Cooper will be conducting a web-based survey on patient safety culture in outpatient areas. The survey will be sent to all Cooper physicians and staff working in outpatient offices.
The AHRQ Medical Office Patient Safety Culture Survey measures organizational conditions that can lead to adverse events and patient harm. Cooper will evaluate the results for common themes so that we may improve patient safety by reducing any adverse events that may cause patient harm within our outpatient areas.

This survey will be administered by ECRI Institute, an independent nonprofit organization dedicated to patient safety that has partnered with Cooper. Targeted physicians and staff will receive the survey via email from ECRI on behalf of Adrienne Kirby, PhD, FACHE, Cooper’s President and Chief Executive Officer, with all the necessary information to complete it.

If you work in more than one location, answer questions according to the office or area where you spend the most of your time. The survey will be open from December 8, 2014, thru January 12, 2015.
This survey is completely confidential. Your opinion counts. Everyone who receives the survey is encouraged to participate.
