CooperUnite’s Assessment-Informed Launch Is Live

CooperUnite is an online tool to help Cooper team members search for and make direct social service referrals for our patients. It is built and powered through a platform called Findhelp, which hosts a nationwide database of more than 17,500 community-based organizations. The tool is currently available through an internet browser (cooperstaff.findhelp.com for team members and cooperunite.org for patients or community members) and is integrated into Epic for providers and myCooper for patients. CooperUnite provides information about essential services such as food, shelter, health care, employment, financial assistance, and more.

This month, Cooper became the third Findhelp partner in the country to launch a new feature, called Assessment-Informed Launch. This feature provides a streamlined experience to allow team members to assess needs and search for resources within the Epic patient chart. After completing a social determinants of health assessment and selecting CooperUnite, providers will see a curated list highlighting the areas of concern or need. This avoids extra time to search, thus enhancing the team member experience and serving our patients with valuable resource information.
