Urban Health Institute Celebrates Second Anniversary

The Urban Health Institute (UHI) and its ambulatory clinical center, the Cooper Advanced Care Center (CACC), are celebrating their two-year anniversary on Friday, June 19, 2015. The UHI and CACC offer innovative ways to reach and care for the vulnerable and underserved population in Camden. In the two years since launching the program, the CACC has increased access to primary care and specialty care while reducing the cost of care. The CACC is located in Three Cooper Plaza, suites 215 and 403 for physician appointments and suite 220 for group visit appointments.

Highlights of the Urban Health Institute:

• Awarded three-year, $2.25M seed-funding grant from The Nicholson Foundation to design innovative ways to care for the underserved.
• Dedicated to serving all self-pay and Medicaid patients and Camden-based Medicare patients.
• Offers primary care plus 22 specialties across medicine, orthopaedics and surgery with 150+physicians and 30+ staff.
• Innovative programs include group medical visits, care management services and nurse-led protocolized visits.
• Focused on data-driven operational stabilization and clinical redesign.
