Congratulations to Rae Easterday, Manager of Voorhees Internal Medicine, for being selected as the Q1 Population Health Quarterly Award Winner. Rae has been instrumental in improving population health outcomes by putting initiatives in place to increase annual wellness visits and training team members to help reach and exceed our goals.
