New, Online Incident-Reporting Tool Coming to Cooper This Week

The Cooper Event and Activity Reporting System (EARS) will be live Wednesday, August 5, 2015.

EARS is an online event (incident), complaint and grievance reporting tool used to improve and enhance safety, quality of care and the patient experience.

Effective August 5, 2015, EARS will be accessible through the Cooper portal home page or desktop shortcut. All reports are confidential and may be entered anonymously. Log on to EARS with your Cooper network username and password.

Below is a list of reportable events:
• Adverse Drug Reaction
• Airway Management
• Blood Product
• Diagnosis/Treatment
• Diagnostic Imaging
• Equipment/Medical Device
• Fall
• Infection
• IV/Vascular Access Device
• Lab/Specimen
• Maternal/Childhood
• Medication/Fluid
• Professional Conduct
• Safety/Security
• Skin Tissue
• Surgery/Procedure
• Tube/Drain

Contact Patient Relations at 856.342.2432 to report any complaints or grievances. They will enter the information into EARS.
