Congratulations to Shelly Daigle-Burns, Population Care Coordinator, on being selected as the Population Health Quarterly Award winner for Q4 2024. Shelly has been instrumental in supporting primary care offices by working with patients for chronic disease care management and goal setting. She has increased the number of patients referred to her and received multiple accolades from providers and patients on her successes. Congratulations, Shelly!
