Just Days Left to Take the Survey to Help With Patient Safety!

One of our goals at Cooper University Health Care is to create a culture of patient safety. For this goal, we have elected to use the Agency for Healthcare Research and Quality’s (AHRQ) well validated and tested Hospital Survey on Patient Safety Culture. As a Cooper employee, you may be chosen to take the survey. If so, you will receive an email to participate in the survey between the dates of July 25, 2016, through August 19, 2016.

The survey is confidential and is being administered by ECRI Institute, an independent, nonprofit organization dedicated to improving patient safety. If you receive an email from ECRI, the email will contain a link to the web-based survey. The survey should take approximately 30 minutes and we ask that you please read the instructions in the email message carefully and complete the survey as soon as possible.

Some of the survey questions may not apply to you. Skip those questions. If you work in more than one location or unit, answer unit-based questions according to the area where you spend the most time.

It is important that we hear from you to evaluate the patient safety culture at Cooper University Health Care. As a result of our last survey, many important improvements were implemented to provide safer care for our patients. Further improvements and initiatives depend on you!

If you have any questions regarding the survey, please contact Linda Futtersak, ECRI Institute Survey Coordinator, at 610.825.6000 ext. 5418 during the business hours of Monday through Friday from 9 a.m. to 5 p.m., EST, or email your questions to lfuttersak@ecri.org.
