Population Health: Cooper’s Employee-Centered Medical Home

Join John F. Robertson, Jr., MD, and the Care Team to learn everything you wanted to know about Cooper’s Employee Centered Medical Home (ECMH). The ECMH is a unique primary care practice for members of the Cooper Health Plan who have complex medical conditions and a willingness to commit to being part of a cutting-edge pilot program in care delivery. During the session, you will hear directly from ECMH patients and learn about their experience in the practice and the quality of care they received through the team-based approach. In addition, Dr. Robertson will share highlights about the practice’s first year and plans for embarking on an exciting year two! With a new round of enrollment about to kick-off, don’t miss this opportunity.
