Effective immediately, physicians’ offices should now call either 888.499.8779 or 856.342.2380 to schedule a patient appointment. The PET/CT Order Form that is required for obtaining pre-authorization for procedures can be found on the portal.
The new form can be found on the portal on the Departments page under Radiology.
