A Groundbreaking Approach to Treating Advanced Melanoma – Now Available at MD Anderson Cancer Center at Cooper

Researchers at MD Anderson Cancer Center at Cooper are leading the way in clinical trials of a new approach to treating advanced melanoma using tumor infiltrating lymphocyte (TIL) therapy.

“TIL therapy is a promising, personalized approach to cancer treatment,” says Young Ki Hong, MD, MPH, surgical oncologist at MD Anderson at Cooper, and principal investigator of several TIL studies.

“Research findings published in top medical journals, such as the New England Journal of Medicine, have shown that TIL therapy not only prolongs the lives of those with advanced melanoma but also allows some to have complete responses, even after failing the previous standard-of-care immunotherapy,” says Dr. Hong.

Dr. Hong, who leads the TIL team at MD Anderson at Cooper, brings unique expertise in this treatment from his prior investigational work with TIL therapy at the National Institutes of Health.

How TIL Therapy Works

After fighting cancer for so long, a patient’s T cells are depleted and no longer effective. TIL therapy replaces these ineffective T cells with the patient’s own boosted, lab-grown T cells that can renew the fight against the melanoma.

First, the melanoma tumor is removed from the patient and sent to a special lab. At the lab, the patient’s cancer-fighting T cells are isolated from the tumor. They are revitalized, grown into billions of cells, and then frozen.

When the new T cells are ready, the patient receives intensive chemotherapy treatment to eliminate regulatory cells, which suppress immune response in the body. Then the boosted, lab-grown T cells are returned to the patient through an infusion. At this point, the only T cells in the patient’s body are their own supercharged cancer-fighting cells. Patients then receive between one and six doses of a medication called IL2, which further boosts effectiveness of the T cells.

The patient’s new T cells regenerate and form lasting memory cells, which continuously look for and attack new cancer cells.

“The research has documented numerous patients who have been cured of their metastatic melanoma for over 10 years from this treatment,” Dr. Hong says.

“The team at MD Anderson at Cooper is proud to be one of the few select centers in the country that can deliver this personalized immunotherapy treatment to our patients. Our team is also taking part and leading clinical trials for the use of TIL therapy for other types of cancer, including metastatic non small cell lung cancer. This therapy has previously been used in treating patients with gynecologic cancers and head and neck cancers.”

Cómo funciona la terapia TIL

Después de luchar contra el cáncer durante tanto tiempo, las células T de un paciente se agotan y ya no son efectivas. La terapia TIL reemplaza estas células T ineficaces con las propias células T potenciadas y cultivadas en el laboratorio del paciente que pueden renovar la lucha contra el melanoma.

Primero, se extrae el tumor de melanoma del paciente y se envía a un laboratorio especial. En el laboratorio, las células T que combaten el cáncer del paciente se aíslan del tumor. Se revitalizan, se cultivan en miles de millones de células y luego se congelan.

Cuando las nuevas células T están listas, el paciente recibe un tratamiento intensivo de quimioterapia para eliminar las células reguladoras, que suprimen la respuesta inmunitaria del cuerpo. Luego, las células T reforzadas cultivadas en laboratorio se devuelven al paciente a través de una infusión. En este punto, las únicas células T en el cuerpo del paciente son sus propias células que combaten el cáncer sobrealimentadas. Luego, los pacientes reciben entre una y seis dosis de un medicamento llamado IL2, que aumenta aún más la eficacia de las células T.

Las nuevas células T del paciente se regeneran y forman células de memoria duraderas, que buscan y atacan continuamente nuevas células cancerosas.

“La investigación ha documentado numerosos pacientes que se han curado de su melanoma metastásico durante más de 10 años con este tratamiento”, dice el Dr. Hong.
