To enhance patient safety, a cross-functional improvement team has evaluated the handoff process between patient transport and clinical staff. After successful pilot programs on Kelemen 8 and in the Emergency Department, we are excited to roll out these changes throughout Cooper University Hospital in Camden.
The following updates are now in effect:
- Ticket to Ride Forms:
- A new “Ticket to Ride” form will be printed from Epic to improve communication during handoffs involving Transport Services. Click here to see a sample.
- These forms will include key transport requirements for improved coordination.
- Epic Resources for Nursing:
- A new “Nurse Transport Report” is available in Epic for nursing roles.
- Reference information supporting the new process will be visible in flowsheets. Click here for more information.
- Updated Transport Policy:
- A new policy, “Transport of Patients,” supporting this change is currently in development.
